Child Protection Report Template

This template assists Educators to document observations, consultations, and actions when they have a concern about a child in the service. These records must provide factual information, observable indicators, and not opinion or conclusions. This template will guide Educators on the information they need to record to ensure all the relevant information is documented.

Please read the Child Protection Report Writing Guide which provides further guidance to assist Educators with writing in a non-judgemental way and is a downloadable PDF which help Educators develop a clear accurate and objective writing style.

A glossary of relevant terms and information is available on the Network site: glossary

When you click the “Submit” button, the form is emailed (in PDF format) to the email address you have provided. It can be saved and kept on the child’s file or attached when making a notification.
